Providers of Color Fair!

Several entities, including the NASW IA Diversity Committee, DCAT, and the IA Department of Human Services, are hosting  Providers of Color Fair on May 3rd, from 11:00 a.m. to 1:00 p.m. at Polk County River Place, 2309 Euclid Avenue in Des Moines. We are featuring programs that provide services to large numbers of minorities and/or provides culturally specific services and/or are providers that are of color. Sponsors and host of the event include DCAT; NASW IA Diversity Committee; University of Iowa School of Social Work, Social Workers of Color; Meskwaki Family Settlement; and Iowa Department of Human Services Minority Youth and Family Initiative. Please come join us for a light lunch and for more information about the providers and agencies that serve our community.

Polk Decategorization and Community Partnerships for Protecting Children (CPPC)  invite you to participate in a Lemonade for Life training.  Lemonade for Life trains professionals working with parents and caregivers in the community on use of the Adverse Childhood Experiences (ACEs) Questionnaire. The goal of the program is to help prevent future exposure to ACEs while promoting resiliency and hope.  If you work in the human services field, this training would be most helpful for training you and your agency/organization staff in learning more about addressing methods for combating the effects of ACEs and Trauma in your everyday work.

The training is being paid for by Polk Decategorization and CPPC, but you are required to register as there are a limited number of available spots for the training.

Training Details: Wednesday, June, 13, 2018
Time:  9:00 am - 4:30 pm (half hour break for lunch)

Complete Training details

Prerequisites:

·  Complete the attached ACEs Questionnaire.  This is for your information only.  We will NOT be collecting these scores.

·  Complete the ACEs 360 Online Training Courses:

1. Go to http://training.dynamiclearningmaps.org/course/view.php?id=91

2.  Login using the following credentials:

Username: acestudy

Password: ACE$tudy1 (password is case sensitive)

There are two modules in the course: The ACE Study: Its Impact and Our Opportunity and A Child Abuse Prevention Response to ACEs. The link to each module is located on the main course page. These modules take 2-4 hours to complete.

 *** Please Note: We are not collecting completion records at this time, so when you are prompted at the end of each section and the end of the course to submit your answers to training questions the program will tell you your answers have not been submitted.  Though we are not collecting completion records, you will need to complete the brief quiz at the end of each section to advance in the program.


 Additional Requirements:

  *   Attend the full training
  *   Post training Coaching Call will be scheduled approximately 45-60 days after Lemonade for Life training
  *   Complete a Post-Training Survey


After completing the Coaching Call AND Post-Training Survey, you will receive:

  *   Certificate of Completion.  This program is eligible for 1.25 (10.25 contact hours) CEU credit.  CEU credit will be

available for an additional charge of $26.30

What is a Family Team Meeting?

A community-based Family Team Decision Making meeting (FTDM) is a facilitated meeting that brings together individuals who can assist families in getting back on track while providing them with helpful resources and supports.

This approach draws upon a family‘s strengths, experiences, knowledge, and resources to create a plan that provides for the safety and well-being the family; especially the children. The family is assisted in building a circle of support that will see the family through difficult times.

FTDMs are based on these values: everyone has strengths, people can change, and people deserve support.

Would Your Family Like to Have a Family Team Decision Making meeting?

You can complete your own referral form and make a self-referral, or you can have someone that you trust complete the referral form on your behalf. The FTDM referral form can be obtained by contacting Polk County’s Project and CPPC Coordinator Jordan Kauffman at jkauffm@dhs.state.ia.us, or by calling 515-725-2787.

During the FTM, the team:

  • Is welcomed and introduces themselves
  • Learns what the family hopes to accomplish and defines the purpose of the meeting
  • Sets reasonable and meaningful goals
  • Recognizes and affirms the family’s strengths
  • Assesses the family’s needs
  • Designs an individualized course of action that includes supports and services matching the family’s needs and building on their strengths
  • Agrees on next steps and sets up supports

In the ongoing Q&A document, click on the specific topic in the Table of Contents to go directly to that topic, and then use CTRL Home to take you back to the Table of Contents.  

REMINDER:  When you download the forms from the CWPTA webpage, you MUST save the documents with the .dot extension in order for them to maintain all functionality of the cells.  In order to do this, complete the following steps:

  • Click on the respective document and open
  • Click on the yellow tab at the top of the document to Enable Editing
  • Under File, select Save As
  • Enter the name of the document behind "File name:"  (This usually defaults to 4701)
  • Save as type:  This defaults to Word Document.  To change, click on the down arrow to the far right and select Word Template from the options listed
  • Choose location, then click Save
